  6. A statement of accuracy and authority, made under penalty of perjury, that the complaining party is authorized to act on behalf of the copyright owner.

Important: Knowingly submitting a false claim of infringement is punishable under 17 U.S.C. §512(f) and may result in civil damages, including costs and attorney’s fees.
